As far as I know, not if you mark it as a gift on the green label that you have to stick on the parcel.

Honestly, I don't know.
I haven't figured out yet how customs work.
So far it seems completely random.
In any case, I do not know anybody in USA to bother for such a thing.

I think it's free up to a certain amount.
I found something that applies to the UK, but the rules for other European countries will be similar (as will be the amounts):
2.3 What are the limits for customs duty and import VAT?

Commercial consignments of £18 or less are free from customs duty and import VAT (please note: With effect from 1 November 2011 the £18 threshold will be reduced to £15 - see 2.4 below). For example, goods purchased over the internet with an intrinsic value not exceeding £18, will not be charged any duty or VAT but this does not include alcohol, tobacco products, perfume or toilet waters. See paragraph 2.6, 2.7 and section 3.
If you are sent a gift with a value of £40 or less, and which complies with the rules shown in paragraph 2.5, it will be free from customs duty and import VAT, but this does not include alcohol, tobacco, perfumes and toilet waters.
Customs duty becomes payable if the value of the goods is over £135 but duty is waived if the amount of duty calculated is less than £9.

Yeah, you just get the ring a little larger, the jeweler will measure your finger and remove a tiny bit of metal from the band. You can't even tell it's been resized.

According to that, in UK I would have to pay since £40 are about $65.

It might be waived if it is less than 9 Pounds Sterling. Thing is, I don't know what the percentage is. VAT is usually charged at around 20% in Europe. 20% of 40 GBP is 8.

So either you have to pay that on top or it might be waived.
Depends on the rules of your country, but judging by the English rules, your taxes and import fees might be around 15 Euros or so.
Depends on how much you want the ring, if it's worth the extra 15 Euros to you.
Easiest thing would be to enquire at customs how much you'd have to pay on an item costing about $65, then you'd know for sure.

In Switzerland, you have to pay VAT on anything over 100 francs. It can get very expensive.

Hm... then it's probably best to check with customs first, before you place your order to avoid unpleasant surprises.

I'd like to have the ring but considering the final price (around $222) plus the resizing (which is not for free I presume)... uhm, I don't know.
Maybe too much expensive.
