I found this report of his passing in a Blog and verified it in SAG.The curious thing,it wasn't reported in either the Starbulletin or Advertiser.

I'm very sad to hear it Sam. Lt. Tankaka is one of my favorite television characters of all time. 
He seemed like a very endearing person, and his life was pretty interesting... born on Maui, studied law at "Boston Law College" (should that be Boston College Law School? maybe not in 1947...), then Duke University. Got into acting in his 40s when he was seen "flirting with two Canadian tourists" (just after he came in from surfing no less) by a Hawaii Five-O casting director. After Magnum ended he was a "Family Court per-diem judge for several years". He lived in an apartment building which he owned in Makiki. (The building is south east of Makiki Cemetery - it's still family owned so I decided against giving the exact location.) Sadly, it seems his son John Soo Wan Lim (2) passed away in 2007 at the age of 55. (Not one hundred percent sure that is his son, but it seems very likely.)
Here's a 2005 Honolulu Star-Bulletin article about Kwan Hi Lim. His passing certainly should have been covered in the Honolulu papers.
